Privacy Policy

Building a questionnaire and downloading a fillable PDF happens in your browser. Questions are not uploaded to our servers, and we do not host completed forms or answers.

Notes you send from the contact page — your name, email, and message — go to a Google Sheet so we can read them and reply. If you leave an email after a download, we store it the same way so we can write when new templates or updates are released. This site uses the Reddit Pixel on visits. Theme preference stays in your browser.

We don’t sell your information. Unsubscribe from any email we send, or ask us on the contact page to delete a stored address.
